【摘要】：目的觀察抗SSA抗體陽性干燥綜合征(p SS)患者血清miR-34、miR-500表達變化,并探討二者在抗SSA抗體陽性p SS診斷中的臨床價值。方法收集p SS患者75例(觀察組),非p SS患者84例(對照組),miRNA芯片分析兩組血清miRNA表達譜改變;并通過實時熒光定量RT-PCR進行驗證。隨后應用統計學方法分析異常表達miRNA在抗SSA抗體陽性p SS診斷中的價值。結果 miRNA芯片分析發現6個上調和4個下調的miRNA,其中miR-34和miR-500表達在觀察組血清中明顯升高;實時熒光定量PCR的結果證實miR-34和miR-500在觀察組明顯上調,與對照組比較,P0.001。受試者工作曲線分析結果顯示,觀察組miR-34對p SS診斷的AUC為0.783(95%CI:0.710~0.844,P0.001);miR-500的AUC為0.774(95%CI:0.701~0.837,P0.001)。而miR-34聯合miR-500在p SS診斷中AUC為0.888(95%CI:0.828~0.932,P0.001);兩者聯合診斷p SS的AUC與單一檢測比較,P均0.05。結論 miR-34和miR-500在抗SSA抗體陽性p SS患者血清中表達升高,檢測血清miR-34和miR-500有助于抗SSA抗體陽性p SS診斷。
[Abstract]:Objective to observe the changes of serum miR-34 miR-500 expression in patients with anti SSA antibody positive Sjogren's syndrome (SSS) and to explore their clinical value in the diagnosis of anti SSA antibody positive PSS. Methods 75 patients with PSS (observation group, 84 patients without PSS) were collected. The changes of serum miRNA expression profiles in two groups were analyzed by miRNA microarray and verified by real-time fluorescence quantitative RT-PCR. Then the value of abnormal expression of miRNA in the diagnosis of anti SSA antibody positive PSS was analyzed by statistical method. Results six upregulated miRNAs and 4 down-regulated miRNAs were found by miRNA microarray analysis, in which the expression of miR-34 and miR-500 was significantly increased in the serum of the observation group, and the results of real-time fluorescence quantitative PCR confirmed that miR-34 and miR-500 were up-regulated in the observation group, compared with the control group (P 0.001). The results of the operating curve analysis showed that the AUC of miR-34 for PSS in the observation group was 0.783 / 95 and the AUC of the observation group was 0.774 / 95CI: 0.701and 0.837 / P0.001respectively. In the diagnosis of PSS, the AUC of miR-34 combined with miR-500 was 0. 888 + 95% CI: 0. 828 and 0. 932% P0.001, and the AUC of the combined diagnosis of PSS was both 0. 05% and 0. 05% compared with that of single detection (P < 0. 05). Conclusion the expression of miR-34 and miR-500 in serum of patients with anti SSA antibody positive PSS is increased. Detection of serum miR-34 and miR-500 is helpful for the diagnosis of anti SSA antibody positive PSS.
